Advocate Physician Partners is pleased to present the 2012 Value Report. The Report highlights the 2011 results of the nationally recognized Clinical Integration Program - a collaborative effort by more than 4,000 physicians and ten Advocate hospitals to drive targeted improvements in health care quality and efficiency.  To learn about how the Clinical Integration Program has evolved over time, click here. 

In 2011, Advocate Physician Partners again achieved record performance in almost all measured areas, resulting in improved patient outcomes and significant cost savings.

Featured Clinical Integration Initiatives

Five key initiatives were the focus of Advocate Physician Partners' 2011 Clinical Integration Program: Generic Prescribing, Asthma Outcomes, Diabetes Care Outcomes, Postpartum Depression Screening and Childhood Immunization. 

Additional Clinical Integration Initiatives

Cardiac Surgery Outcomes, Controlling High Blood Pressure and the Pupulation Health Wellness Initiative (Adult and Pediatrics) are among the 52 additional initiatives Advocate Physician Partners physicians participated in during 2011.
