If you are planning your delivery at Advocate Lutheran General Hospital, preregistration can be completed well in advance of your due date. Choose from:

Whether you choose to preregister online or complete our paper form, you wil be asked for these items upon admission:

• A copy of your photo identification
• A copy of your health insurance card (front and back)

Choose a pediatrician
During admission you will be asked to provide the name of your pediatrician so that we may notify him or her as soon as your baby is born. If your pediatrician or family practice physician is not on staff at Lutheran General Hospital, your new baby will be cared for by an on-call pediatrician during your stay.
