At Advocate Physician Partners, eligible patients are enrolled in our Patient Outreach Program which provides disease management educational information to patients as well as regular reminders to follow up with their physician for recommended care.

Information is distributed to the patient on behalf of his or her physician through mailings and phone calls. The program utilizes a variety of materials including pamphlets, postcards, magnets and pocket cards.
