Gifts to Advocate Home Health Services bring clinical expertise into patients’ homes, providing quality, comprehensive health care throughout the Chicagoland metropolitan area.
Asthma Initiative
Helping asthmatics breathe easier
The Asthma Initiative seeks to develop a consistent and aggressive approach to the management of asthma across the system. Your gift will help provide screenings, education and outreach programs.
Baby Advocate Program
Giving parents the tools for a healthy start:
Baby Advocate is Advocate Health Care’s answer to the urgent problem of Chicago’s low immunization rates. This system-wide program gives to parents of babies born at Advocate hospitals access to valuable resources and support, such as reminders for check-up and immunization dates. Your gift helps support age appropriate child health newsletters, incentives for completion of recommended immunizations, and access to online and telephone referrals for the families of our newest patients.
Center for Pediatric Research
Developing tomorrow's care techniques:
The Center for Pediatric Research provides the expertise and the infrastructure to support pediatric clinical studies and research programs. Needed infrastructure includes research consultation, identification of study sites, budget and contract negotiation, regulatory documentation, internal review board submission, coordination of ancillary services, participant recruitment, data management and central interface with the study sponsors.
Childhood Trauma Treatment Program
Helping children overcome sexual abuse:
Advocate’s Childhood Trauma Treatment Program, part of the Family Care Network, treats both children and adults who have been victims of sexual abuse and sexual abuse offenders. Your gift helps treat some of the most at-risk and abused children in our community, and provides community outreach and education to clinical professionals and community members affected by an epidemic of abuse.
Congregational Health Partnerships
Working within faith communities to meet health needs:
Funds contributed to this priority enable Advocate Health Care to tap into the healing work of congregations. Through this network, Advocate can help congregations develop or build on existing health ministries, link them to resources, and broker partnerships between faith communities and organizations that promote health and wellness.
Dreyer Community Health Fund
Making care accessible for Fox Valley residents:
The Dreyer Community Health Fund promotes the health and well-being of the Fox Valley area by providing much-needed funding and support to health care organizations that target underserved populations. For instance, your contribution supports free clinics that provide lifesaving prescriptions to people who could not otherwise afford them.
Electronic Intensive Care Unit
Creating an extra layer of monitoring for critical patients:
The electronic intensive care unit (eICU) provides around the clock centralized video/audio monitoring by Advocate physicians and nurses for our most critical patients in our intensive care units. This technology supplements the already high level of care provided by our hospitals and will enhance the quality of care we provide to our patients. The eICU save lives, significantly reduces complications, and reduces the length of a patient’s stay in an ICU. The contributed funds will help to purchase equipment, conduct research, and create an advanced training program for personnel to utilize this ground-breaking capability.
Healthy Steps for Young Children
Helping new parents meet their children's needs:
The system-wide program Healthy Steps for Young Children uses a national model to enhance opportunities for primary care physicians to interact with parents and children 0-3 about child development issues. Your contribution helps Healthy Steps specialists to prepare physician practices to work more effectively with families around the challenges of early childhood.
Heart Failure Initiative
Helping patients manage a dangerous condition:
Your contribution to this system-wide program helps to reduce re-admissions, increase appropriate use of medications and enhance patient management of congestive heart failure.
Mission and Spiritual Care
Treating the whole person in mind, body and spirit:
Funds contributed to this project will allow Advocate Health Care to move Spiritual Care forward in integration, research, prevention of illness and spirituality in health care settings.
Parish Nursing
Treating the whole person in mind, body and spirit:
An integral part of the Parish Nurse Ministry are the Inner City Ministries. Funds to this program place parish nurses in congregations located in some of Chicago’s most vulnerable communities and medically disadvantaged areas. As advocates for care, parish nurses are often the only primary health care link for their parishioners. They connect at-risk members of the congregations to existing local community resources and help those in need by providing basic health care services, health counseling and health advocacy.
Reach Out and Read
Encouraging family interaction, health and literacy:
Reach Out and Read is a national program that seeks to make early literacy an integral part of pediatric primary care, and strives to decrease illiteracy rates and promote healthy family activities. Your gift buys age-appropriate books for pediatric patients to take home at all pediatric check-ups from six months to five years of age, and encourages parents to share reading time with their children.
Senior Advocate
Equipping seniors to manage their care:
Senior Advocate provides health resources and customized wellness programs for seniors. Your gift helps offer a number of benefits, including financial counseling, insurance counseling, information referral, discounted and/or free screenings and more.
Trauma Initiative
Improving emergency care by sharing best practices: Funds contributed to this priority will allow Advocate Health Care to integrate the delivery of trauma care among Advocate's four trauma centers through clinical coordination, clinical and patient/family education, information system development, clinical and outcomes research and community partnerships.
