
Trinity Partners with Chicago Family Health Center
Advocate Trinity Hospital is pleased to announce a collaboration with Chicago Family Health Center (CFHC), a non-profit, community-based federally qualified health center. Trinity and CFHC will work together to eliminate the overuse of the hospital's emergency department for non-emergency issues by operating an on-site health center near to the front entrance of the hospital The health center will be staffed with board-certified CFHC physicians, licensed nurses, medical assistants, patient service representatives and social service staff. All CFHC physicians have admitting privileges at Trinity Hospital.
Patients who come to the Emergency Department will register and be seen by a triage nurse, just as they are now. The triage nurse will assess each patient and determine the necessary level of treatment. Patients whose conditions are not of an emergent nature will be given the options of waiting to be seen by a physician in the emergency room-where patients with more urgent symptoms will take priority-or being seen at the CFHC health center.
If any follow-up visits are required, patient who choose to see a CFHC physician will do so at one of the CFHC health centers in the South Chicago, Pullman, and Roseland communities.
