We are a community-based group practice providing services for babies, children and teens. At Advocate Health Center-Evergreen Plaza, we listen and respond to your needs and share the Advocate values of treating every patient as an individual with dignity and respect.

Our Office Hours:
Monday-Thursday: 8 a.m.-8 p.m.
Friday: 8 a.m.-5 p.m.
Saturday: 9 a.m.-1 p.m.

Special Services
For your convenience, on-site services include laboratory and health education. Appointments can also be made with specialists in neurology.

Hospital Care
If hospital services are needed, your physician can refer your child to Advocate Hope Children's Hospital or Little Company of Mary Hospital.

Baby Advocate
Baby Advocate is another exclusive program that's free to mothers who deliver their babies at an Advocate hospital or with an Advocate physician. Baby Advocate provides support, such as health information and classes, to mothers and their babies from birth until age two.
