We are a community-based group practice providing services for babies, children, and teens, adults and seniors. At Advocate Health Center-Logan Square, we listen and respond to your needs and share the Advocate values of treating every patient as an individual with dignity and respect.

Our Office Hours:
Monday and Tuesday: 8:30 a.m.-7 p.m.
Wednesday and Thursday: 8:30 a.m.-6 p.m.
Friday: 8:30 a.m.-5 p.m.
Saturday: 8:30 a.m.-12:30 p.m.

Special Services
For your convenience, on-site service include laboratory. Appointments can also be made with specialists in obstetrics and gynecology. Spanish speaking medical and support staff are available.

Hospital Care
If you need hospital services, your physician can refer you and your family to Advocate Illinois Masonic Medical Center.
