We are a community-based group practice providing services for babies, children, and teens, adults and seniors. At Advocate Health Center-Six Corners, we listen and respond to your needs and share the Advocate values of treating every patient as an individual with dignity and respect.

Our Office Hours:
Monday & Tuesday: 8 a.m. - 7 p.m.
Wednesday, Thursday & Friday: 8 a.m. - 5 p.m.
Saturday: 8 a.m. - 1 p.m.

Special Services
Spanish-speaking and Polish-speaking medical staff and associates are available.

Hospital Care
If you need hospital services, your physician can refer you and your family to Advocate Illinois Masonic Medical Center.
