Congestive Heart Failure Program Congestive Heart Failure (CHF) affects nearly 4.8 million people in the U.S. and is a major cause of admission to the hospital for people 65 and older. CHF symptoms can become worse over time if the disease is untreated. Advocate Health Center CHF patients can reach their highest level of health and reduce their risk factors through a special CHF care management program. A registered nurse, working along with an Advocate Health Center primary care physician, helps patients learn how to balance medications, diet and activity. Patients in the program receive a follow-up visit as needed with the program nurse to monitor and discuss their treatment plan and support groups to help them cope with their illness. With the proper balance of medicines, diet, exercise and clinical support, heart failure can be effectively managed.
