Alzheimer's disease Memory Assessment Centers

Advocate Memory Assessment Centers offer a comprehensive evaluation and functional assessment to aide in the diagnosis and treatment of Alzheimer's disease and other types of dementia. Their goal is to assist patients, families and care partners throughout the trajectory of the disease.  The assessment process includes all or some components of the following:

• A comprehensive physical examination by a geriatric specialist 
• Battery of basic blood tests to rule out alternative causes for changes in memory
• Brain imaging (e.g., MRI, CT or PET scan)
• Neuropsychological testing
• Social Work evaluation and supportive counseling
• Comprehensive care conference with the patient, family, care partner and the treatment team to discuss findings and develop an individualized plan of care
• Collaboration with the primary care physician to report all findings

Patients may be referred for consultation by their primary care physicians or may contact the office directly with questions or to schedule an appointment.
