Premier Training Program for Family Medicine Physicians

• We provide medical school graduates with advanced training in the specialty of Family Medicine. Resident physicians are actively involved in direct patient care. They work closely in teams with our Family Medicine attendings to insure continuity and quality care for all patients. The program is affiliated with University of Illinois School of Medicine.

Special Services

Comprehensive evaluation and functional assessment for memory loss and Alzheimer's disease At the Memory Assessment and Alzheimer’s Center, our goal is to help identify a diagnosis and possible treatment that can assist patients and their families to cope with memory loss, Alzheimer’s disease, and related areas of concern. We provide a thorough comprehensive evaluation that may include some or all of the following components:

• Initial contact by our social worker to address patient and family concerns and provide care giver support
• A physical examination conducted by one of our geriatric medicine specialists
• Laboratory and brain imaging studies
• Consultation with a neuropsychologist for a more detailed evaluation of memory function
• Care conference held with the patient, family, and treatment team to devise an individualized plan of care
• Monthly Caregiver Support Group sponsored by the Alzheimer’s Association facilitated by our licensed clinical social worker

We aim to improve the health risk of and quality of life for our patients by prescribing individual therapeutic lifestyle recommendations and educating individuals on implementing those recommendations while providing support during the change process.

Special Services

• Diabetes Education: Advocate Medical Group’s Diabetic Education Program, which has been recognized by the American Diabetes Association and has received the NCQA physician recognition, is now offering diabetes classes. Classes focus on techniques to help manage blood glucose, including nutritional guidance and individual meal plans. You will also have the opportunity to learn about the latest in diabetes technology. To enroll in a class call 847-318-2400.
• Diabetes Support Group: If you’ve had diabetes for 12 weeks or 12 years, taking care of everything can be a stressful job. AMG’s diabetes educators hold a support group on the 4th Thursday of each month in the lower level at the Nesset Pavilion to help all individuals with diabetes cope with their disease.
