Acceptance in Advocate Physician Partners is based, in part, on the practice needs of the individual Physician Hospital Organizations (PHO). Opportunities for Primary Care Physicians are generally available but do vary by site. For this reason, both Primary Care Physicians and Specialists are encouraged to contact us to discuss current opportunities.
Please note that all applicants must be board certified or board-eligible (residency completion date and date of board exam must be provided) at the time application for membership is made.
If you are not currently on the medical staff of an Advocate Health Care hospital and have not applied for medical staff privileges, we encourage you to contact the Physician Relations team at the hospital in which you are interested.
If you are on the medical staff of an Advocate hospital or have submitted your application (not the pre-application) for medical staff privileges, please contact the Provider Relations Specialist for the PHO to which you wish to apply.
General Membership Criteria
Physician members must be board certified or board-eligible and are expected to adhere to the policies and standards set forth by Advocate Physician Partners. The table below summarizes some of the general requirements. Please discuss any questions or concerns with the appropriate PHO Director.

Primary Care Physicians: Panel Size
|
Before closing panel, Internal Medicine Physician must be assigned a minimum of 200 Managed Care Members; Family Practice and Pediatricians must be assigned a minimum of 300 Managed Care Members.

Access and Availability
|
Standards as set forth in the provider handbook.
|
|
Physician must be a member of the PHO at each hospital where he/she is a member of the medical staff.
|
|
Electronic Data Interchange (EDI) & Internet Connectivity
|
Physician must have electronic connectivity with our Operations team to facilitate data exchange.
|
|
All physicians must have high-speed internet access in office.
|
|
Physician must submit all claims through EDI.
|
|
Contract Participation
|
Physician must participate in HMO Risk contracts through APP.
|
|
Physicians who elect to participate in clinically-integrated fee-for-service contracts (i.e. PPO, POS, etc.) that are negotiated by APP must participate through APP for all such contracts or other products negotiated by APP.

|
Clinical Integration Program Participation
|
Physician must participate in the APP Clinical Integration programs.
|
|
