Physical Therapy, Speech Therapy and Occupational Therapy
Philosophy:
Physical Medicine as a health and rehabilitation profession provides services to persons of all ages with the ultimate goal of restoring the patient's ability to function to their optimum potential and within the constraints of their disease and injury. The department is committed to the application of purposeful, goal-oriented activity in the evaluation, assessment and treatment of persons whose function is impaired by physical illness or injury or the aging process.
Types and Ages of Patients served:
The department provides inpatient and outpatient care to children, adults, and geriatric patients.
Physical Medical Services Provided:
The following services are available for the disciplines of physical Therapy, Occupational Therapy and Speech Therapy.
- Patient Evaluations/Assessments
- Developing and implementing a treatment plan/program for the patient
- Periodic re-evaluation/re-assessments of patients and modifying treatment program as needed
- Patient/family/caregiver and associate education
- Discharge planning
- Inter-disciplinary collaboration of services with Nursing, Continuity of Care, Dietitian, Physician, Pharmacist, Utilization Review, Home Health Care Coordinator
Hours of Operation
The Physical Medicine Department is open 8:00 a.m. to 4:30 p.m. Monday through Saturday. The Physical Therapist is on call on Sunday's for evaluation/assessments. For more information or to schedule an appointment, please call 773.265.3667.
