Adopt-A-School Program

Program Purpose: To establish partnerships between local schools, families, and area health care providers to promote community health awareness and prevention.

Adopt-a-School Program System:

Components:

• Health Provider and School nurse complete application to establish needs and services.
• Health Provider and School Nurse sign linkage agreement.
• School Nurse supplies no-compliance or needs list (i.e. children who need immunizations, have tooth aches, or excessive and/or unusual behavior disturbances).
• School Nurse and Health Provider coordinate phone calls or means of contact of appointments.
• Health Provider provides services at a nominal fee.
