Advocate Trinity Hospital was built with the leadership of our neighbors more than a century ago and it continues to rely on the generosity of its donors. Charitable gifts are vital to helping us change lives and save lives — whether it is advancing our service offerings such as cardiovascular or stroke care, reaching out to the community through programs such as free HIV testing in the emergency room, or bringing health education to schools and senior centers.

Below is a list of programs at Trinity Hospital that currently need your support.
Asthma Community Outreach Asthma is a chronic disease that causes pain, loss of productivity and even death, but it can be self-managed. Gifts to this program help educate emergency room patients how to recognize triggers and avoid attacks. Philanthropy also supports an outreach program design to teach students and teachers about asthma.
Cardiovascular Services Trinity Hospital now provides minimally invasive diagnosis, treatment and rehabilitation for cardiovascular disease to meet the growing needs of its community. With your support, Trinity Hospital will be able to offer more sophisticated and rapid diagnosis, non-surgical treatment for more cardiovascular conditions and advanced rehabilitation services.
Chronic Kidney Disease Management Because many people do not know they have a kidney problem, Trinity Hospital has established a chronic kidney disease management program. Your gift will help Trinity Hospital offer free kidney screenings, intervention and education that will empower people to manage the health of their kidneys.
Diabetes Education This program partners health care providers and community members to improve the treatment and outcomes for people with diabetes, to promote early diagnosis and ultimately to prevent the onset of diabetes. Advocate Trinity Hospital's comprehensive approach includes classes for newly diagnosed individuals, an ongoing support group and diabetes screening and risk assessment activities. Gifts to this program provide educational tools and resources, particularly to address the needs of Latino and African-American populations.
Healthier Communities Initiatives Most programs included in this initiative are preventive outreach to address the needs of underserved populations on the south side of Chicago. These are often offered in partnership with faith communities, schools and other community organizations.
Nursing Education and Resource Center Patients benefit when nurses grow professionally. The center is a place for gaining the information and capabilities they need to serve their patients.
Patient Transportation Van Trinity Hospital's van service provides free transportation to and from the hospital campus for seniors from the communities we serve. Your gift to maintain this service makes health care accessible to seniors whose limited mobility could keep them from getting the care they need.
Stroke Services Advocate Trinity Hospital's service area has the highest incidence of stroke in the city of Chicago. As the main care provider in the community, Trinity Hospital is committed to providing the best treatment options for stroke victims. Your gift will help Trinity Hospital ensure that community members recognize the signs and symptoms of strokes through outreach programs and education.
Women's Health Initiatives Gifts to this program ensure that South-Side women of all ages have access to early diagnoses for breast cancer. This program educates women about the importance of early detection and strives to decrease the length of time between an abnormal mammogram, final diagnosis and treatment planning-for better mental health as well as faster recovery.
President's Fund for Special Needs Giving to the President's Fund for Special Needs at Trinity Hospital provides additional resources and flexibility to respond quickly to new ideas and programs that support the hospital's strategic directions.
