In 2011, each of Advocate Health Care’s hospitals began a three-year process to conduct a hospital-specific Community Health Needs Assessment (CHNA). Advocate’s hospitals summarized this activity in hospital-specific CHNA Reports in December 2013. These reports are provided below for members of the community to view, print, read and provide feedback, if desired, in compliance with the Patient Protection and Affordable Care Act (PPACA).
