In 2011, each of Advocate Health Care’s hospitals began a three-year process to conduct a hospital-specific Community Health Needs Assessment (CHNA). Advocate’s hospitals summarized this activity in hospital-specific CHNA Reports in December 2013. These reports are provided below for members of the community to view, print, and read. There is also an option to provide feedback via a link in the CHNA report, if desired, in compliance with the Patient Protection and Affordable Care Act (PPACA).
