Entering Into Partnership

Each Educational Institution must sign a legal contract with Advocate Health Care and Good Samaritan Hospital before placing students in our setting. Obtaining the Clinical Affiliation Agreement can sometimes be a lengthy process so it is recommended that initiation begin as soon as possible. Although securing the agreement is the first step in starting our partnership, it does not guarantee a clinical rotation.

Clinical Affiliations Handbook: Contains information about Advocate Healthcare and Good Samaritan Hospital with directions and campus map. Also provides key information regarding our Standards of Behavior, Culture of Safety, Joint Commission and National Patient Safety Goals, Risk Management, and Infection Control practices. Page 31 contains the Acceptance of Clinical Affiliations Handbook Form that is required to be signed by all students and faculty.

Faculty Guide: Describes the faculty requirements and responsibilities. Also contains a Faculty Check List to facilitate completion of all requirements.
