The social worker collaborates with the team to formulate a safe patient discharge plan, evaluate each patient and develop an individualized plan. This plan may include, but is not limited to, follow-up therapy, medical equipment and community services/resources. The social worker will work closely with each patient and loved ones to assist with the transition to the next level of care.
