The Challenge

At Advocate Health Care, we offer a disability management program for both short term and long term non-work-related injury and illness. It is our goal to maximize your employee benefits while ensuring quality care. We work with your employee and their physician to facilitate a prompt, safe return to work. This allows you to maintain a neutral position relative to your disabled employee. In addition, we conduct medical reviews, assess appropriate length of time off, maintain contact with all care providers, and offer restricted duty, job modification and Americans with Disabilities Act consultation services.

A closer look at our management process:
Active communication
Active communication is maintained among your employee, their treating physician, their manager and human resources department ensuring:

	Employee confidentiality while you are kept informed of restrictions, accommodations and work status
	Ongoing written documentation to your employee’s manager indicating their disability status
	Ongoing correspondence among your employee, their case manager and treating physician with regard to their current treatment plan

Multidisciplinary approach and clinical determination of your employee’s job readiness
A successful disability management program is lead by a clinical determination of your employee’s job readiness and utilizes active involvement of your employee’s:

	Treating physician
	Registered nurse case manager
	Vocational rehabilitation specialist
	Occupational health physician, and
	Additional health care providers as necessary

Individual case review
We look at each of your disabled employees’ cases individually, taking into consideration their:

	Previous medical history
	Current health status
	Lifestyle
	Demographics

Job readiness training program
In the event your employee is unable to return to his/her previous job, we offer a job readiness program including:

	Vocational evaluation
	Enhancing their interviewing skills
	Support and guidance throughout their job search
