MyAdvocateEureka Patient Portal
MyAdvocateEureka is a free, password-protected, web-based service that allows Advocate Eureka Hospital patients to securely access portions of their electronic medical record related to hospital visits, procedures and tests. In the near future, this hosptial portal will be fully integrated with the Advocate Medical Group patient portal to enhance the coordination of care for those who have services from both AMG providers and the hospital.
What can you do with MyAdvocateEureka?
With MyAdvocateEureka, you can view the following information from your Advocate Eureka Hospital medical record:
- Appointment listings
- Discharge instructions & vital signs
- Certain test results
- Home medications
If you're also a patient at Advocate BroMenn Medical Center in Normal, you can access this information from your BroMenn medical record in same portal.

What information can I see in MyAdvocateEureka?
You can see information from your medical record, such as laboratory and radiology test results, home medications, allergies, medical history, current health issues and due dates for preventive care. You may also print a Health Summary for your records.
Is there a fee to use MyAdvocateEureka?
No! MyAdvocateEureka is a free service offered to our patients.
How is MyAdvocateEureka secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. MyAdvocateEureka uses SSL encryption technology. All MyAdvocateEureka messaging is done while you are securely logged on to our web site.
How do I sign up?
When visiting Advocate Eureka Hospital you will be asked if you would like to enroll in our patient portal. If you decline at that time, or would like to enroll and are not planning a visit in the near future, you must do the following: Fill out a Release of Information Form in person at the Medical Records department at Advocate Eureka Hospital. You must have a personal email address. Once the form has been received and processed, you will receive an email detailing next steps. Processing normally takes 48 business hours after our office receives the completed form.
Can I also see my family members' health records?
In certain cases. This is called Proxy Access. For a child under 12, this allows a parent or legal guardian to log into their personal MyAdvocateEureka account, and then view information regarding their child. Once a child turns 18, a parent can only access the child’s patient medical record information with the patient’s authorization. It is also possible to request access to another adult’s health record if you help manage that adult’s medical care. For more information please contact our Patient Portal Hotline at 309.268.5600.
How do I see my test results?
Your test results and reports are both available by clicking on the Health Record button on the portal home page. Once you click on the Health Record button you will be taken to the Health Record page where you will see buttons on the right labeled Results and Reports where you will have access to your Laboratory Results and Other Reports.
How do I get the complete set of my medical records?
You can obtain your medical records by visiting the Medical Records department at Advocate Eureka Hospital.
Some of my information is wrong. How do I correct this?
If you notice an error in your personal information, such as address and phone number, please send a change request from the portal. The change request can be found by clicking Profile, and then Update Profile. If you see information in your medical record information that is incorrect, please contact our Patient Portal Hotline at 309.268.5600.
How can I update my email address in MyAdvocateEureka?
To change the email address that MyAdvocateEureka uses for sending activity, password reset, or other communications, please contact our Patient Portal Hotline at 309.268.5600.
How do I communicate my medical concerns?
MyAdvocateEureka is not used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 911 IMMEDIATELY.
