Advocate Good Shepherd Hospital is committed to working with our patients and their physicians to reduce the risks of diabetes-related complications. We offer our patients the latest information and technology as well as the opportunity to receive their education in a comprehensive and coordinated way.
Diabetes Self-Management Program
- Individual Education Counseling
Individual sessions for Prediabetes, Type 1, Type 2 and Gestational Diabetes. These sessions include a comprehensive assessment of individual needs. Sessions consist of appointments with both a Registered Nurse and a Registered Dietitian. Topics include: self-monitoring of blood glucose, nutrition, diabetes care techniques, medication review and more.

- Comprehensive Class Overview
This Medicare-approved program will cover a variety of topics in three two-hour classes which will be offered on three consecutive days. Topics include: diabetes overview, understanding medications, healthy eating, foot care, stress management, coping, changing behavior, and exercise. These programs are offered monthly.

This community education series covers a variety of medical-related topics to assist patients in managing and understanding diabetes and its complications. While this list is not all-inclusive, our speakers include an endocrinologist, a cardiologist, a podiatrist, a dentist, a neurologist, and a nephrologist.

- Diabetes Support Group (LIVING WELL WITH DIABETES)
Learn to innovatively live with diabetes through the experience and support of others. This monthly support group is offered for those with diabetes, their family and friends. Join us for 90 minutes of sharing and education.
