Congestive heart failure (CHF) is a condition in which the heart muscles can no longer pump enough blood to the rest of the body. This can cause a number of problems throughout the body including organ damage resulting from lack of oxygen and blood flow. This condition does not mean that the heart
has completely failed and will stop. It does mean that it needs help to work better.

- Daily follow-up contact by a nurse assigned to each CHF patient to check on weight, answer questions and help with diet and activity decision making
- CHF management education, including nutrition, exercise, medication instruction and more
- Customized cardiac rehabilitation program designed to help patients recover, reduce risk factors, prevent future illness and provide emotional support
- CHF Support Group to assist in the management of heart failure
