Whether you choose to preregister online or complete our paper form, you will be asked for these items upon admission:
- A copy of your photo identification
- A copy of your health insurance card (front and back)

Choose a pediatrician
During admission you will be asked to provide the name of your pediatrician so that we may notify him or her as soon as your baby is born. If your pediatrician or family practice physician is not on staff at Good Shepherd Hospital, your new baby will be cared for by an on-call pediatrician during your stay. Prior to discharge, we will need the name and phone number of the pediatrician or family practice physician you will use for follow-up care.
