The Certificate of Medical Necessity is the preferred order form for diabetes education services that includes all of the criteria for Medicare approval and reimbursement. Please use this order form when referring patients for diabetes education services. Make sure that the order includes the following:
- Patient name and date of birth
- Date of order
- Diagnosis and co-morbidities
- Current labs
- Training ordered
- Barriers to learning
- MD signature

The Blood Glucose Results log provides you and your health care providers an overview of your blood glucose results so accomplishments and difficulties can be quickly identified. The Food and Activity Diary can be used to log your daily food intake and daily exercise. |
