At Advocate Illinois Masonic Medical Center, patient safety and quality of care are important to us. As part of Advocate Health Care, a faith-based, non-profit organization, our mission to deliver quality, compassionate care begins and ends with meeting the needs and expectations of our patients. We encourage you to contact us about any concerns you may have about patient care and safety at our hospital.
We take your concerns seriously. For that reason, Advocate Illinois Masonic Medical CenterTM leadership team asks that you contact them directly to discuss any issues or questions. Please call a Patient Relations Representative, Monday through Friday, 8:00am - 5:00pm, at: 773.296.8230. Let the Patient Relations Representative answering your call know that you have a concern about patient care and safety that you need to discuss.
After normal business hours, please call our hospital operator, at: 773.975.1600, and ask to speak with the nursing supervisor on duty. They can communicate your concerns to us during off hours, so we can respond promptly and work to resolve your issue.

Approximately two weeks after you are discharged from the hospital, you may receive a survey asking you to evaluate your experience here at Advocate Illinois Masonic Medical Center. One of our top strategic initiatives is service excellence; it is a visible way of ensuring we are providing the best possible experience for our patients. Your comments will help us learn if we are fulfilling our values. We strongly encourage you to complete and return the survey so that we can continue our mission of service excellence.
