Cardiac Rehabilitation

Having a heart attack or being diagnosed with a heart condition can be life-changing. But it doesn't have to mean the end of healthy, active living. In fact, diagnosis and treatment are just the first steps in dealing with a heart condition.

Cardiac Rehabilitation for Better Patient Outcomes

At Advocate Illinois Masonic Medical Center, we believe that cardiac rehabilitation is an important part of recovery after a serious heart event. The goal of our cardiac rehabilitation program is to help patients avoid further heart events and repeat hospitalization by improving their overall heart health.

Our dedicated cardiac rehabilitation specialists work closely with you to help you stick with the program and meet your health goals. To support you in your rehabilitation, we offer:

  • An individualized exercise program designed by professionally trained rehabilitation nurses and exercise physiologists
  • Flexible scheduling to adjust for work and personal schedules
  • Orientation addressing transportation, family support, cost and patient commitment issues
  • Staff support and encouragement, both on-site and by phone
  • Information and education on risk factor identification and modification
  • Patient buddy system to help acclimate new patients
  • Dietary guidelines and support
  • Language interpreters available by advance request

Three-Phase Cardiac Rehabilitation Program

We offer a comprehensive, fully accredited three-phase cardiac rehabilitation program aimed at helping cardiac patients resume and maintain a healthy lifestyle.

Phase I: Beginning Your Cardiac Recovery
Phase I of cardiac rehabilitation is an important first step toward recovery that generally begins while you are still in the hospital and ends when you are discharged. With your physician’s orders, our rehabilitation staff will assess your physical abilities and recommend the next steps toward recovery, including education about how to care for yourself after discharge. We’ll follow up with weekly phone calls to check on your progress until you’re ready for Phase II.

Cost: Included in your hospital bill

Phase II: Working Toward a Healthy Lifestyle
Adopting a healthy lifestyle is key to sustained recovery. Phase II cardiac rehabilitation usually begins two to six weeks after hospital discharge with a referral from your physician. This phase starts with an assessment by our staff, who will then develop a rehabilitation program customized for you. The program will include:

  • A 12-week/36-session outpatient progressive rehabilitation program
  • 90-minute classes held on Mondays, Wednesdays and Fridays in the medical center’s Cardiac Rehab Department
  • Close monitoring of your response to progressive exercise
  • Educational focus on personal risk factor control (diet, exercise, stress management, smoking cessation, weight loss, etc.)
Cost: Many insurance companies including Medicare cover all or most of the program’s cost. Please check with your insurance carrier on the extent of your coverage.

Phase III: Managing Your Cardiac Health Independently
At this phase of rehabilitation, individuals become more independent in managing their exercise, while continuing to benefit from counseling, education and staff oversight. Monitoring is available but not required. Individuals exercise two to three times per week indefinitely. Physician referral is required.

Adherence to the program provides lifelong benefits, including:

  • Greater exercise tolerance
  • Increased heart capacity
  • Lower blood pressure and cholesterol
  • Better eating habits
  • Improved stress management skills

Cost: Phase III is only covered by a few insurance plans and is otherwise self-pay.

Getting Started

Individuals are welcome to participate in our rehabilitation program with a referral from their physician, whether or not they have received care at Illinois Masonic Medical Center. The program benefits anyone who has had:

  • A heart attack
  • Heart bypass surgery
  • Coronary angioplasty
  • Coronary artery bypass surgery
  • Stable angina
  • Coronary artery disease
  • Congestive heart failure
  • A pacemaker (implantation)
  • Heart valve surgery
  • Other heart conditions
Ask your primary care physician or cardiologist for a referral to the appropriate phase of rehabilitation.

Parking is available in the Wellington Parking Garage. Valet parking is available at the hospital’s main entrance from 6 a.m. to 7 p.m., Monday through Friday.

We are also a short walk from the Wellington L station and CTA buses.
