What to Bring With You
Whether you choose to preregister online or complete our paper form, you will be asked for these items upon admission:
- A copy of your photo ID
- A copy of the front and back of your health insurance card, if you have one
Before You Come to the Hospital, Choose Your Baby’s Doctor
When you are admitted, you will be asked to provide the name and phone number of your baby’s doctor so we may notify him or her as soon as your baby is born.
If your pediatrician or family practice physician is not on staff at Illinois Masonic Medical Center, your new baby will be cared for by an on-call pediatrician during your stay.
