How to Become a Volunteer

There are three easy ways to schedule an interview to become an Advocate Lutheran General Hospital volunteer.

  the hours of 8 a.m. and 5 p.m.,
  Monday through Friday
• E-mail us at

  (You will be asked to sign your application at your interview)
• Print out, complete and mail or fax application to:

Volunteer Profile / Application Form

Upon receipt of your application, we will contact you within five business days for a brief phone interview. At this time, we will invite you to a new volunteer orientation that acquaints you with the hospital's important policies and procedures.

All volunteers are required to complete a health assessment form that will be given to you at the time of your orientation. The employee health department also administers a TB test; there is no charge to the volunteers.

After completing this process, you will be ready to begin volunteering at Lutheran General Hospital!
