The Anticoagulation Center is a warfarin management service, which is staffed by clinical pharmacists, who are trained in anticoagulation monitoring. Warfarin is an anticoagulant, or anti-clotting drug, which helps to prevent harmful blood clots from forming in the body. Blood testing is required to determine the correct warfarin dose.

Patients at all stages of anticoagulation therapy are welcome, including: patients who are new to warfarin, unstable on their warfarin dose, have mobility or transportation issues, have compliance issues, have collapsed veins or trouble with blood draws, or need extended before-or-after work hours.

The center offers the following services:

• Finger-stick INR blood testing with immediate results
• Patient education on anticoagulation
• Management of diet interactions
• Management of drug interactions (prescription, over-the-counter, herbals)
• Warfarin dose adjustment by a clinical pharmacist
• Instructions for interruptions in therapy, with enoxaparin teaching when necessary

• Deep venous thrombosis (DVT): blood clot in the leg
• Pulmonary embolism (PE): blood clot in the lung
• Atrial fibrillation (afib): irregular heart beat
• Cerebral vascular accident (CVA): stroke
• Transient ischemic attack (TIA): mini-stroke
• Heart valve replacement surgery

Referral information

All patients must have a referral on file, which is signed by the physician responsible for the anticoagulation therapy. Physicians making a referral can call the Anticoagulation Center 847.723.2345 and request a referral form be sent to their office, or simply download a referral form and fax to 847.723.2328.
