Helpful hints for your first appointment
Your first appointment at Advocate Lutheran General Hospital Bariatric and Metabolic Center will be filled with valuable information about how you can prepare for weight-loss surgery. You will meet with the nurse/program coordinator, dietitian, exercise physiologist and one of the surgeons. This initial appointment is approximately three hours long.
Prior to your first appointment, please complete the following:

- Complete the new patient form.
- Make a copy of your insurance card and driver's license.
Please return your new patient form along with a copy of the front and back of your insurance card to our office prior to your initial consultation. Information can be faxed to 630.261.9335.
Once we receive your packet of information, we will verify your benefits and contact you.
