How Shared Savings Are Distributed

Providing the full spectrum of health care services within the Advocate Health Care Network is consistent with the stated purposes of the Medicare Shared Savings Program and the mission of Advocate Health Care.  In-network care coordination provides more efficient and effective care for our patients.  As such, the governing body of APPAC, Inc. has determined that, in order to further the interests of the ACO as a whole in meeting the objectives of the Medicare Shared Savings Program, it is reasonably necessary for APPAC to include an in-network care coordination component and care management engagement factor in the Advocate Physician Partners Clinical Integration Program.  The in-network care coordination component and care management factor will be made available to all eligible APPAC ACO participants and ACO providers and suppliers.
