General Office E-mail Correspondence
Consents, patient literature and HIPAA waiver requests
are to be sent as email attachments per protocol submission
checklist. When the IRB receives the protocol submission,
the principal investigator, or the person listed on the
application as a study contact, will be notified via email
regarding the IRB protocol number and any missing materials.
All future correspondence should be submitted to the IRB office
under the assigned protocol number.
