The proxy process allows patients to receive electronic access to a family members MyAdvocate patient portal account.
If you would like to become a proxy to another patient’s account, or assign a proxy to help manage your own account, please print and fill out the proxy consent form and provide it to the office registration clerk during your next office visit. You will also need proper identification and any legal paperwork necessary if requesting access to an adults account. It is necessary that both the patient and the proxy have an account on file with Advocate Medical Group to establish a proxy relationship.
Due to Illinois State law, patient accounts will be disabled between the ages of 12 and 17. During this time, parents or legal guardians will not have proxy access into the minor’s chart.
Please note that once the consent form is received it could take several business days to complete the request.
