We maintain contracts with all major health plans in the Chicago metropolitan area and select plans in downstate Illinois. These contracts represent 230,000 capitated lives in Health Maintenance Organization plans and an estimated 700,000 Preferred Provider Organization patients.  As a member of Advocate Physician Partners, you'll enjoy a number of benefits related to our contract portfolio:

Medical Management

Practice management has become a complex and time-consuming task.  As a physician-led organization, we support your desire to focus on practicing medicine by assisting the physician practice with day-to-day functions such as credentialing, eligibility and referral, utilization and case management services. Clinical Integration and Pay-for-Performance

In an environment of increasing demand for performance data, cost savings and improved health outcomes, Advocate Physician Partners physician members are uniquely positioned to succeed.  Our nationally-recognized Clinical Integration Program provides the infrastructure and support physicians need to succeed in the post-reform world of medicine.

The Advocate Physician Partners' Clinical Integration Program is a nationally recognized model of care, uniting independent and employed physicians.  The Program is built on standards set by industry leadership groups like the Centers for Medicare and Medicaid Services (CMS), the National Quality Forum (NQF), the National Committee for Quality Assurance (NCQA) and the American Medical Association (AMA).  These measures serve as the gold standard for measuring provider performance and managing population health status.

Technological Support

Health care technology represents a tremendous investment for any practice.  Members have access to a suite of tools designed to streamline office processes and improve patient safety. 

Electronic Referral Management Application (ERMA) streamlines the referral process and helps ensure patients receive appropriate care based on clinical need and health care coverage. Embedded protocols provide clinical decision support rooted in evidence-based treatments and best practices. 

CIRRIS Point-of-Care® integrates three critical functions that previously were accessed through standalone applications: patient registries, generic prescribing data and physician progress reports. It enables physicians to track their progress in the Clinical Integration Program and make adjustments to improve performance. 

Electronic Medical Record (EMR) - recent health care reform legislation has created new urgency for the adoption of an EMR by all physicians.  As a physician member, you'll have access to a comprehensive system designed to improve patient safety and clinical outcomes. 

Electronic Intensive Care Unit (eICU®)- implemented in 2004 by Advocate Health Care, the eICU® harnesses cutting-edge technology for round-the-clock, real-time audio and video monitoring of intensive care unit patients in eight Advocate hospitals. Patients in Advocate's 18 adult ICUs receive care from highly trained Intensivist physicians based in a central command center.
