It is recommended that schools submit Clinical Rotation requests SIX months prior to the start of the clinical rotation. Submitting a request DOES NOT GUARANTEE A CLINICAL ROTATION.

Student Clinical Request Form:

• All requests must be submitted on the Student Clinical Request Form and sent via email to the address listed below.
• All information requested on the form must be completed. 
• Confirm the accuracy of the Clinical Rotation dates!  Verify that the requested dates correspond to the actual days of the week requested.
• Faculty IS REQUIRED TO REMAIN "ON-SITE" during the clinical rotation. 
• Course description or syllabus, outlining the students' clinical expectations, must accompany each request.

Graduate Student Clinical Requirements Form:

• All requests must be submitted on the Student Clinical Requirements Form-Graduate Student and sent via email to the address listed below.
• All information requested on the form must be completed. 
• Course description or syllabus, outlining the students' clinical expectations, must accompany each request.
• The Student Nurse Orientation PowerPoint MUST be completed prior to the start of the Clinical rotation. It may be completed in the Center for Research, Education, and Development on the first day of the clinical rotation.
