School Requirements:
Once a clinical rotation is approved and scheduled, the following form and information must be submitted a minimum of two (2) weeks prior to the rotation start date. Information must be emailed to: darina.lichner@advocatehealth.com
The program coordinator/Dean must verify that all students and faculty/instructors have met the health requirements and documents are on file with the school.
NEW FOR FALL 2011: All students and faculty must have proof of receiving the flu vaccination prior to the start of the clinical rotation. Any medical or religious exceptions must be approved by our employee health department. Students or faculty who do not comply with this will not be able to participate in the rotation. Verification must be received in our office 24 hours prior to the students starting on site.
