
Resources for Health Ministry Development
Many resources exist to support wellness programming in congregations. Advocate Community Ministry owns many of these resources for the purpose of sharing them with congregations. A Resource Library houses books, videos, activities, and "ready-to-use" programs. Several different Health Promotion Programs that are specifically designed for use with congregations are available. Resources for Health Ministry Development include trainings and pre-packaged programs that congregations can use as they develop their health focus or program.
Resource Library
Advocate Community Ministry has collected some of the best resources available for health ministry programs and makes them accessible to congregations through a library housed in the ACM office. Congregations may check materials out with a valid, current credit card. The library is also open for anyone to visit during working hours, Monday through Friday in the Community Ministry office at 205 Touhy Ave, Park Ridge. Please call ahead to make sure someone is available to assist you. You may also order materials over the phone or via email. The resource center includes books, videos, tapes and articles about health and healing in congregations as well as a range of "ready-to-use" tools and programs.

Health Promotion Programs
The Advocate Community Ministy office can help congregations implement several different health status assessment programs, a health values clarification program and other similar initiatives. Most of these will be free or very reasonably priced. These programs may also be mixed and matched to suit the unique interests of your congregation.
Health Status Assessment Programs
Comprehensive Congregational Health Assessment
This comprehensive health status assessment program encourages people to take responsibility for their health by integrating their faith teachings with good personal health practice, keeping their own health records and clarifying their values around health and wellness. Participants have key diagnostic lab work taken, complete a Health Behavior Assessment questionnaire, are weighed, measured and have their blood pressure recorded. After the results are returned, participants receive a Personal Health Record notebook with results from their assessment and other health record information. Educational programs are scheduled throughout the year to address health issues identified through the assessment process. At the end of the year, the tests are repeated so that participants can see if they have improved their health. Data for the entire congregation is also available. There is a fee for this program.

Age-Specific Health Behavior Assessment
This is a pencil and paper assessment of behaviors in numerous wholistic health categories, including physical, safety, nutrition, emotional, relational, environmental, intellectual, social and spiritual. An individual report summarizes the results and provides a list of resources for improving health in each area. Aggregate results are also provided for the group. Tests are available for children, adolescents, adults and older adults.

Personal Health Record
This notebook encourages individuals to take responsibility for their own health by keeping track of important medical information in one place. The notebook is divided into sections including health values, important phone numbers, personal data, health history, screening/test results, health inventory results, meeting with the doctor, suggested questions for the doctor, and end of life issues. Individuals take this book with them when they visit the doctor, enter the hospital or travel. Usually this notebook is used along with one of the health status assessment programs. There is a small fee for the personal health record notebook.

Health Values Clarification Program
This program leads participants through a process of reflecting on their beliefs and values around what it means to be well, end-of-life issues, perceptions of health care providers, thoughts about independence and control, overall attitude about life, relationships, spirituality, etc. It can be used as a study series, small group activity or as part of a retreat.
