area, the organization is able to meet the needs of the entire nine county
Advocate Health Care service areas. Advocate Home Health Services is part of
Advocate Health Care, one of the top health care systems in the country and the
largest fully integrated health care delivery system in Illinois.

- Consists of three service categories:
- Advocate Home Health Services, Inc:
Short-term visits (also called intermittent services) for skilled nursing,
therapies, medical social work and disease management
- Advocate Hospice:
Nursing, chaplain, medical social work, equipment, pharmaceuticals,
bereavement services and respite care offered to patients with terminal
conditions
- Advocate Home Care Products, Inc:
Home Infusion Pharmacy Program, provides nutrition and pharmaceuticals
delivered via infusion therapy and also provides skilled nursing and Home
Medical Equipment and Respiratory Therapy Program, provides oxygen
therapy, a sleep program, other respiratory equipment, beds, wheelchairs,
and ambulatory aids
Key Accomplishments
- Recipient of disease-specific care certification by Joint Commission on the
Accreditation of Healthcare Organizations asthma and heart failure management
- Performance levels exceed 10 of 11 home health quality measures published for
Medicare certified home health agencies by Home Health Compare (2004) and
the state average in the same 10 indicators
- Exceeds state and national re-hospitalization rates by 10.7 percent
Community Commitment and Philanthropy
Advocate Home Health Services has a strong focus on philanthropy and the well
being of patients and their families.
- Volunteer and Bereavement programs offers support for patients and families
participating in the hospice program
- Volunteer hours rose to 33% in 2004; bereavement service counseling and
community service increased 53%
- Advocate Hospice Family fund supports Hospice families with children

- Encounter Technology Solutions (ETS), a hand-held computer used by field
staff to record and access patient data
