Residency Application

Instructions for Completing the Application Process:

1. Complete and return the application form.
2. Send a copy of your CV and a passport-size photo.
3. Forward a copy of your College of Pharmacy transcripts.
4. Distribute the recommendation forms to three professional references (pharmacy employers and/or clinical instructors). Be sure references are aware of the deadline.
5. You will be contacted regarding on-site interviews after all applications have been reviewed.
