HF Reminders
Keep track of your weight:

* Weigh yourself at the same time every day. Wear the same type of clothing. Weigh after urinating and before eating.
* Write down your weight each day on the weight chart. Bring the chart to your doctor appointments.

Call your Doctor right away if:

* You gain 2 or more pounds in 1 day.
* You gain 3 to 5 pounds in 1 week.

Other reminders:

* Take your medicine as your doctor tells you.
* Refill your prescriptions before you run out of medicine.
* Stay on your low salt diet.
* Follow the activity guidelines developed by you and your doctor.

Call your Doctor if:

* Your symptoms get worse.
* You get tired faster.
* You have dizzy spells or faint.
* Your feet or ankles swell more than usual.
* Breathing becomes more difficult or you start coughing at night.
* You begin urinating less.
* You have tightness or pain in your chest.
