We are also available to answer your questions about benefits and help you review your options regarding Medicare and supplemental insurances to allow you to make choices that are best for you. 	Information Referral—A dedicated representative is available to help you access hospital and community resources. Information is available on programs across the United States for those caregivers whose relatives are not living close to them. Call 847.723.7770.
	Adult Day Care—A safe, secure setting for older adults who, because of physical or mental disabilities, require supervision during the day. Call 847.723.7770.
	Home Delivered Meals—A support service provided to homebound elderly or handicapped who cannot prepare their own meals. Call 847.723.7770.
	Geriatric Assessment—An assessment performed by professional staff to help determine cognitive and functional abilities. Call 847.723.7700.
	Consultation—A form of short-term crisis intervention consisting of one to three interviews to help families experiencing problems with elderly family. Call 847.723.7770.
	Counseling—Provides emotional relief and support to older adults and/or caregivers. Call 847.723.7770.
	Support Groups—Caregiver support groups meet monthly at the Adult Day Care Center. Call 847.723.7770.
	Emergency Response Units—A system for those home alone, who have health risks or concerns, that calls for help with the simple push of a button. Call 847.723.7770.
	Discounted Fitness Center Membership—All seniors (age 65 and older) are eligible to receive a discounted membership to the Lutheran General Fitness Center.
	Educational Seminars and Health Fairs—Periodic seminars on a variety of topics related to your health and personal safety.
	Senior Advocate Magazine—A quarterly health and wellness magazine with articles and events of interest to seniors.

How to Join Senior Advocate
To become a Senior Advocate member, fill out the enrollment form. You may either submit the form online or print and fill out a copy and mail it to the address on the form. For assistance in completing this application, call 1.800.323.8622 Monday through Friday, from 8 a.m. to 6 p.m.
