Physicians at Advocate Trinity Hospital provide the full spectrum of services to elderly patients. We offer a comprehensive approach to the complex problems of aging, including memory disorders, frailty and weight loss, gait and balance disorders, osteoporosis and incontinence.
Patients undergo annual geriatric evaluations that involve physical, sensory, cognitive and affective testing to determine appropriate treatment plans. Our physicians consult with urologists, rehabilitation therapists, neurologists and geriatric psychiatrists to aggressively treat and resolve reversible conditions, manage chronic diseases, stabilize functional problems and focus on future prevention and fitness.
Our patients are encouraged to participate in outpatient sub-acute physical therapy as opposed to inpatient services, which contributes to longer term mobility and independence. Physicians also follow up with patients on-site at nursing homes and provide specialized wound treatments, including artificial skin replacement.
We are focused on patient and family education to promote optimal health and improve quality of life. When dealing with end of life issues, we understand that loved ones can be too attached to the patient to recognize the need for palliative care. Our physicians and nurses are specially trained to administer palliative care and have developed relationships with inpatient hospice programs, should the need arise.
