Three-phase Cardiac Rehabilitation Program

Advocate Trinity Hospital’s three-phase Cardiac Rehabilitation Program is a fully accredited program designed to help patients get back on their feet safely following a cardiac event. The program’s knowledgeable medical professionals work closely with patients to help them regain strength and reacquire the skills needed to lead an active, healthy lifestyle. We combine individualized, monitored education, lifestyle modification techniques and diet and exercise plans to guide patients to a healthy future.

Phase I: Beginning Your Recovery
Phase I of cardiac rehabilitation is an important first step toward recovery, that generally begins while you are still in the hospital. With your doctor’s approval, experienced rehabilitation professionals on staff at Trinity Hospital provide education and supervised physical activity. When you are ready to go home, our cardiac reconditioning specialists work with rehabilitation personnel to develop a personalized program for your continued recovery after discharge.

Phase II: Working Toward a Healthy Lifestyle
Adopting a healthy lifestyle is key to a sustained recovery. To help individuals in their efforts, Phase II provides a personalized program that focuses on education, exercise and counseling for patients who have returned home. This 12-week phase includes a progressive exercise program, custom designed for you by an exercise physiologist. Guided exercise, which may include walking on the treadmill, riding a stationary bike or working with free weights, will help you restore strength, endurance and confidence. Participants typically exercise three times a week at Trinity Hospital’s state-of-the-art exercise facility. Fully equipped with EKG and blood pressure monitors, the center provides continuous monitoring of individuals to ensure safety.

Personal counseling and education provided by rehabilitation staff provide the tools to help individuals build a healthier lifestyle.

Family members are encouraged to attend educational sessions to gain a better understanding of their role in supporting their loved one. Transportation is available for a small fee for program participants living within a five-mile radius of Trinity Hospital. To arrange transportation, call 773-967-2000 extension 4100.

Phase III: Managing Your Program Independently
At this phase of rehabilitation, individuals become more independent in managing their exercise, while continuing to benefit from counseling, education and EKG checks. Adherence to the program provides life-long benefits, including:

  • Greater exercise tolerance
  • Increased heart capacity
  • Lower blood pressure and cholesterol
  • Better eating habits
  • Improved stress management skills

Getting Started
Individuals are welcome to participate in our rehabilitation program with a referral from their physician, regardless of whether they have received care at Trinity Hospital. The program is of great benefit to those who have experienced:

  • A recent heart attack
  • Heart bypass surgery
  • Angioplasty and/or stent
  • Stable angina
  • Coronary artery disease
  • Other heart problems

Primary Physician Involvement
Cardiac rehabilitation personnel will stay in regular contact with your physician throughout your participation in the rehabilitation program. Your physician will receive progress reports, and consult on care provided by the program staff.

Insurance Coverage
The cost of this program is covered by most health plans. To ensure coverage, please check with your insurance provider about your individual benefits.
