Your health share coverage

Advocate Health Care or Aurora Health Care does not accept health share coverage.

Because you recently scheduled an appointment with us, we want to keep you informed. We’re unable to submit claims on your behalf because we don’t accept your coverage.


Here are options to get you the care you need:

  • Cancel your appointment with us: Please cancel in LiveWell or contact the Advocate or Aurora provider or location you intended to visit. We also encourage you to reach out to your health share plan to determine your options.
  • Keep your appointment with us: If you keep your scheduled appointment with us, we’ll require a signed Financial Responsibility Agreement at the time of your visit, in which you agree to pay for your service. Depending on your service, we may collect a deposit or require you to pay for your visit prior to your appointment.
