  • Did you receive a referral? If your Medicaid Managed Care plan referred you to us, contact the Advocate or Aurora provider or location you intended to visit for more information.
  • If not, please cancel your appointment with us: Cancel in LiveWell or contact the Advocate or Aurora provider or location you intended to visit.
  • Contact your insurance company: We encourage you to ask your insurance company to help you find an assigned in-network provider or organization.
