SMS terms & conditions – care management

When you sign up for text messages from Advocate Health Care, you are signing up to receive text messages related to your relationship with Advocate Health Care and affiliates. “Affiliates” is in reference to Advocate Health Care’s sub-healthcare providers, and all messaging will be controlled by Advocate Health Care. Messaging could include updates related to your visits, LiveWell or MyChart account, one-time passcode, billing notifications, prescription reminders and care management.

Opt-in information

By opting in, you provide consent to receive text messages using an automatic telephone dialing system. When you opt in to the service, we will send you a message to confirm your sign-up. You may receive SMS mobile text alerts by several methods.

Text START to 57269 to receive text messages related to your relationship with Advocate Health Care and affiliates, including updates related to your visits, LiveWell or MyChart account, one-time passcode, billing notifications, prescription reminders and care management.

Opt-out information

You can opt out of SMS messages by texting STOP to the respective short code.

Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from.

If you want to join again, sign up using LiveWell or text HELP to the short code for instructions.

Mobile carriers

Carriers are not liable for delayed or undelivered messages. Successful delivery is affected by a number of factors including accuracy of the mobile number you provide, sufficient network capacity, your ability to be in the range of a transmission site, etc. We cannot be liable for losses or damages arising from delayed, misdirected or nondelivery. It may also be possible that the full contents of the message may not be delivered to your phone.

Message and data rates may apply to messages we send you, or you send to us. Message frequency may vary. If you have questions about your personal text or data plan and potential charges, it is best to contact your cellular provider.
