Chicago, Illinois – Citywide, the number of deaths caused by diabetes-related complications is 59.5 percent. Yet, the death rate from diabetes is 79.5 percent among Southeast Side residents - specifically those living within Chicago’s 60617 zip code, which has an 80 percent Hispanic population.
With funding from a new $58,000 grant from the Chicago CARES to Prevent Diabetes collaborative, Advocate Trinity Hospital will expand its successful T2 Diabetes Prevention program to Hispanic adults who share the same 60617 zip code with the hospital or live nearby. This free, community program teaches participants how to prevent or reduce their risk for Type 2 diabetes through wise food choices, physical activity and other changes for a healthier lifestyle.
"There is an urgent need to bring diabetes education and lifestyle coaching to the Hispanic community," said Regional Community Health Manager Erika Davis.
The T2 Diabetes Prevention program for Spanish- and English-speaking Hispanic adults will launch:

Funds from the Chicago CARES grant will be used to underwrite a variety of educational aides and program incentives, including the availability of Spanish-speaking lifestyle coaches, as well as grocery, gym memberships, acupuncture and transportation assistance to reduce barriers to exercise and healthy food choices.
In September of 2017, Advocate Trinity launched the T2 Diabetes Prevention Program, targeted toward African Americans living within and nearby the 60617 zip code. To date, a total of 34 participants enrolled in the diabetes prevention program with 29 participants completing an average of 14 sessions. Each participant saw an average weight loss of 2 percent. Advocate Trinity’s T2 Diabetes Program is recognized by the Centers for Disease Control and Prevention National Diabetes Prevention Program.
